ID3KO.M5CAR is an experimental chimeric antigen receptor (CAR) T-cell therapy engineered to target mesothelin-expressing solid tumors while resisting the functional exhaustion typically induced by chronic antigen exposure. Developed by researchers at the University of Pennsylvania and the University of California San Francisco, this construct utilizes CRISPR/Cas9 technology to knock out the transcription factor ID3 (Inhibitor of DNA Binding 3) in mesothelin-redirected CAR T cells (M5CAR). Preclinical studies have identified ID3 as a key regulator of the dysfunctional 'NK-like' T-cell phenotype and epigenetic exhaustion signature that limits the efficacy of CAR-T therapies in solid tumors. By disrupting ID3, the therapy aims to maintain superior cytotoxicity, reduce the expression of exhaustion-related genes, and improve the persistence of the CAR T cells within the tumor microenvironment. Preclinical models of pancreatic cancer have demonstrated that ID3KO.M5CAR T cells exhibit enhanced anti-tumor activity compared to standard mesothelin-targeted CAR T cells.
