Idasanutlin + cytarabine is a combination regimen of the small molecule MDM2 inhibitor idasanutlin and the antimetabolite chemotherapy cytarabine, investigated primarily for treatment of acute myeloid leukemia (AML), including relapsed or refractory (R/R) AML and newly diagnosed AML not suitable for standard induction regimens. Idasanutlin inhibits murine double minute 2 (MDM2), blocking MDM2-mediated degradation of the tumor suppressor protein p53, thereby promoting p53-dependent apoptosis in cancer cells, particularly those with TP53 wild type status. Cytarabine (also known as Ara-C) is an antimetabolite that inhibits DNA synthesis, causing cell death, especially in rapidly dividing hematopoietic cells. The combination was designed to enhance anti-leukemic activity by synergistically promoting cell cycle arrest and apoptosis. Clinical trials (including MIRROS, NCT02545283) have assessed this combination; however, development in R/R AML was terminated due to insufficient benefit over cytarabine alone[2][3][4][5][6].
