idecabtagene vicleucel + talquetamab is an investigational combination regimen for the treatment of relapsed or refractory multiple myeloma, particularly in patients exposed to at least three major classes of therapy. Idecabtagene vicleucel is a genetically modified autologous CAR-T cell therapy that targets the B-cell maturation antigen (BCMA) on myeloma cells, leading to targeted T-cell-mediated cytotoxicity. Talquetamab is a bispecific monoclonal antibody that targets the G protein-coupled receptor class C group 5 member D (GPRC5D) on myeloma cells and CD3 on T cells, redirecting T-cell cytotoxicity against GPRC5D-expressing cells. Both agents have shown efficacy as monotherapies but are being studied in combination (phase 1/2) to overcome resistance mechanisms and improve outcomes in highly refractory multiple myeloma. The regimen leverages two distinct immunotherapeutic approaches: CAR-T cell therapy and bispecific T-cell redirection.
