Overview

Idelalisib + nab-paclitaxel is an experimental combination therapy consisting of idelalisib, an oral small molecule inhibitor of phosphoinositide 3-kinase delta (PI3Kδ), and nab-paclitaxel (nanoparticle albumin-bound paclitaxel), an intravenously administered antimicrotubule agent. Idelalisib targets PI3Kδ, which is important for the function of lymphocytes, leading to apoptosis and inhibition of signaling in malignant cells[3][1]. Nab-paclitaxel works by promoting tubulin polymerization and stabilizing microtubules, thereby preventing mitotic processes and inhibiting cell division[2][4]. This combination has been investigated for the treatment of metastatic pancreatic ductal adenocarcinoma, specifically in previously untreated patients, but the trial was terminated early and efficacy endpoints were not reported[1]. Idelalisib is developed by Gilead Sciences and nab-paclitaxel by Abraxis BioScience (now part of Celgene/Bristol Myers Squibb)[1][2][4]. This combination is not an approved or established regimen and is considered investigational.
