This is a combination therapy consisting of an isocitrate dehydrogenase 1 (IDH1) R132H mutation-specific peptide vaccine and the alkylating chemotherapy agent temozolomide. The IDH1 peptide vaccine is designed to stimulate an immune response specifically against tumor cells expressing the mutant IDH1 protein, which is a hallmark of many low-grade gliomas. The mechanism involves induction of T cell responses targeting the neoepitope created by the R132H mutation in IDH1, leading to anti-tumor immunity. Temozolomide acts as a DNA alkylating agent that induces cytotoxicity in dividing tumor cells and may also enhance immunogenicity by increasing antigen presentation or modulating the tumor microenvironment. This combination has been investigated primarily for recurrent grade II glioma with confirmed mutant IDH1 status, aiming to improve immune-mediated tumor control alongside standard chemotherapy[2][5][6].
