Drug intelligence / Profile preview
IDH1R132H-vac (also known as IDH1-vac or MIDH1-vac) is an investigational peptide-based cancer vaccine designed to target the tumor-specific IDH1 R132H neoantigen, a mutant form of isocitrate dehydrogenase 1 commonly found in diffuse gliomas, including astrocytomas and oligodendrogliomas. Developed by Heidelberg University and the German Cancer Research Center (DKFZ), the vaccine is a 20-mer peptide that is processed and presented on MHC class II molecules, eliciting a robust mutation-specific CD4+ T helper cell and humoral immune response. It has been evaluated in several Phase 1 clinical trials, including NOA16 (newly diagnosed astrocytomas), AMPLIFY-NEOVAC (recurrent gliomas, in combination with the PD-L1 inhibitor avelumab), and iVOCAL (recurrent astrocytomas, in combination with the dual IDH1/2 inhibitor vorasidenib and topical imiquimod adjuvant).
