The idiotype mRNA vaccine is a personalized therapeutic cancer vaccine designed to treat B-cell lymphoma by targeting the unique idiotype of the B-cell receptor (BCR) expressed on clonal tumor cells. Developed by researchers at Stanford University, the vaccine utilizes mRNA encoding the heavy and light chain variable regions of the patient-specific BCR. To enhance antigen presentation, the construct includes an MHC class I trafficking domain (MITD). The mRNA is formulated with charge-altering releasable transporters (CARTs), a polymeric delivery system that releases the mRNA within antigen-presenting cells. When co-administered with a TLR9 ligand adjuvant (such as CpG), the vaccine induces a robust polyclonal T-cell response, including cytolytic CD8+ T cells, leading to significant tumor growth reduction in preclinical models.
