The idiotype single-chain variable fragment (scFv) vaccine is a patient-specific active immunotherapy developed for the treatment of follicular B-cell lymphoma. Produced using a transient plant-virus expression system (Tobacco Mosaic Virus vector), the vaccine consists of the recombinant VH and VL domains of the patient's unique tumor-specific immunoglobulin (idiotype). Unlike traditional idiotype vaccines that require complex hybridoma-based production and chemical coupling to carrier proteins like keyhole limpet haemocyanin (KLH), this scFv subunit design is intended to simplify manufacturing and rapidly induce a polyclonal, tumor-specific immune response. In clinical trials, it has been administered subcutaneously, often in conjunction with the adjuvant GM-CSF, to stimulate both humoral and cellular anti-tumor immunity by targeting the unique B-cell receptor idiotype present on the surface of malignant B cells.
