Drug intelligence / Profile preview
IDP-023 + isatuximab is an investigational combination regimen consisting of IDP-023, an allogeneic, off-the-shelf natural killer (NK) cell therapy, and isatuximab (Sarclisa), an anti-CD38 monoclonal antibody. Developed by Indapta Therapeutics in collaboration with Sanofi, this combination is designed to treat relapsed/refractory multiple myeloma (RRMM), particularly to rescue patients who have become resistant to CD38-directed therapies. IDP-023 utilizes a specialized subset of naturally occurring NK cells known as g-NK cells, which are epigenetically reprogrammed following cytomegalovirus exposure to lack the FcεRIγ adapter protein and the inhibitory checkpoint NKG2A, while expressing high levels of the activating receptor NKG2C. This phenotype enables enhanced antibody-dependent cell-mediated cytotoxicity (ADCC) upon CD16 engagement and potent, antibody-independent killing of HLA-E-expressing tumor cells. Isatuximab binds to CD38 on malignant plasma cells, inducing direct apoptosis and mediating immune-effector mechanisms such as ADCC, which are significantly amplified by the co-administered g-NK cells. The combination is currently being evaluated in a Phase 1/2 clinical trial.
