The ICE regimen is a combination chemotherapy protocol consisting of ifosfamide, cisplatin, and etoposide. It is widely utilized as an induction chemotherapy for various malignant germ cell tumors, including those located in the central nervous system (CNS). Ifosfamide is an alkylating agent that interferes with DNA replication by forming cross-links. Cisplatin is a platinum-based antineoplastic agent that causes DNA damage by creating intra-strand and inter-strand cross-links, thereby inhibiting DNA synthesis. Etoposide is a semisynthetic derivative of podophyllotoxin that acts as a topoisomerase II inhibitor, preventing the re-ligation of DNA strands and leading to double-strand breaks and apoptosis. Beijing Tiantan Hospital is currently evaluating this three-drug combination in a Phase II clinical trial for patients with localized basal ganglia germ cell tumors, specifically as induction therapy followed by whole-ventricle irradiation.
