Overview

This is a combination chemotherapy regimen consisting of five agents: ifosfamide, etoposide, methotrexate, vincristine, and prednisone. Each drug has a distinct mechanism of action targeting cancer cells through different pathways: - Ifosfamide is an alkylating agent that crosslinks DNA strands, inhibiting DNA replication and leading to cell death. - Etoposide inhibits topoisomerase II, causing DNA strand breaks during the S and G2 phases of the cell cycle. - Methotrexate is an antimetabolite that inhibits dihydrofolate reductase (DHFR), blocking folic acid metabolism required for DNA synthesis. - Vincristine binds to tubulin and disrupts microtubule formation, arresting cells in metaphase during mitosis. - Prednisone is a synthetic glucocorticoid with lympholytic effects; it induces apoptosis in certain cancerous lymphocytes. This specific combination does not have a widely recognized acronym or brand name but represents an intensive multi-agent chemotherapy protocol. It has been studied primarily for relapsed or refractory Hodgkin's disease and high-grade non-Hodgkin lymphoma[1][7]. The regimen aims to maximize cytotoxicity by combining drugs with complementary mechanisms while minimizing overlapping toxicities.
