XNW5004 + abiraterone + prednisone is an investigational combination regimen consisting of the oral small molecule EZH2 inhibitor igermetostat (XNW5004), the CYP17A1 inhibitor abiraterone, and the corticosteroid prednisone. Igermetostat targets Enhancer of Zeste Homolog 2 (EZH2), an epigenetic regulator whose overexpression is linked to poor prognosis and treatment resistance in metastatic castration-resistant prostate cancer (mCRPC). Abiraterone blocks androgen biosynthesis by inhibiting the CYP17 enzyme complex, while prednisone is co-administered to manage mineralocorticoid excess associated with abiraterone treatment. Developed by Evopoint Biosciences, this combination is being evaluated in Phase Ib/II clinical trials for its potential to overcome resistance to androgen receptor pathway inhibitors in patients with advanced prostate cancer.
