igermetostat + CHOP

XNW5004 + CHOP (extended) is a combination regimen featuring igermetostat (XNW5004), a selective, substrate-competitive small molecule inhibitor of EZH2 (Enhancer of Zeste Homolog 2), and the standard CHOP chemotherapy regimen (cyclophosphamide, doxorubicin, vincristine, and prednisone). Developed by Evopoint Biosciences, this regimen is primarily investigated for the treatment of newly diagnosed peripheral T-cell lymphoma (PTCL). The "extended" designation refers to a specific clinical trial arm (NCT05161533) where patients who achieve stable disease or better following the initial combination cycles continue to receive igermetostat as maintenance therapy. EZH2 is a histone methyltransferase and the catalytic subunit of the Polycomb Repressive Complex 2 (PRC2), which is often overexpressed or mutated in various hematologic malignancies, leading to gene silencing and tumor progression. By inhibiting EZH2, the regimen aims to restore the expression of tumor suppressor genes and enhance the cytotoxic effects of the CHOP chemotherapy backbone.
