IgM-enriched intravenous immunoglobulin (Pentaglobin) is a polyclonal biologic preparation containing a mixture of human immunoglobulins, specifically enriched with the IgM fraction (approximately 12%), alongside IgA (12%) and IgG (76%). Manufactured by Grifols, it is primarily used as an adjunctive therapy for sepsis and septic shock. The drug's mechanism of action leverages the pentameric structure of IgM to effectively neutralize bacterial exo- and endotoxins, activate the complement system, and enhance opsonization and phagocytosis. It also exerts immunomodulatory effects by scavenging excess complement factors and modulating the production of sepsis mediators. Current clinical research, such as the Phase 3 IgM-fat trial sponsored by Azienda Ospedaliero-Universitaria di Modena, is evaluating personalized dosing strategies based on serum IgM titers to improve outcomes in patients with septic shock.
