Overview

IHL-42X is an oral fixed-dose combination drug developed for the treatment of obstructive sleep apnea (OSA). It consists of two active pharmaceutical ingredients: dronabinol, a synthetic form of delta-9-tetrahydrocannabinol (THC), and acetazolamide, a carbonic anhydrase inhibitor. Dronabinol acts by binding to cannabinoid receptors to stimulate airway dilation, while acetazolamide induces mild metabolic acidosis that increases respiratory drive by signaling excess CO2 in the blood. The combination is designed to synergistically target multiple aspects of OSA pathophysiology, including intermittent hypoxia, hypercapnia, and upper airway muscle tone. Clinical trials have shown that IHL-42X can significantly reduce the Apnea-Hypopnea Index (AHI) and improve sleep quality with good tolerability. The drug is being developed by Incannex Healthcare and is currently in Phase 2/3 clinical trials targeting patients who are non-compliant or intolerant to positive airway pressure therapies[1][2][3][5][6][7].
