IL-1β-pretreated mesenchymal stem cells (βMSCs) are a modified cell therapy product where human umbilical cord-derived mesenchymal stem cells are stimulated with the pro-inflammatory cytokine interleukin-1β (IL-1β) prior to administration. This pretreatment enhances the therapeutic efficacy of the MSCs by upregulating the expression of miR-146a, an anti-inflammatory microRNA, which is then selectively packaged into exosomes. These exosomes are transferred to macrophages, promoting their polarization toward an anti-inflammatory M2 phenotype. In preclinical studies, βMSCs have demonstrated superior ability to ameliorate symptoms and increase survival in murine models of sepsis compared to naïve MSCs, suggesting a potential modality for treating severe inflammatory disorders.
