Overview

IL-18 armored anti-U5 snRNP200 CAR T cells are an experimental, preclinical-stage chimeric antigen receptor (CAR) T cell therapy developed by Memorial Sloan Kettering Cancer Center (MSK) for the treatment of acute leukemias, including acute myeloid leukemia (AML) and B-cell acute lymphoblastic leukemia (B-ALL). The therapy targets U5 snRNP200, an RNA helicase typically found in the nucleus that aberrantly translocates to the cell surface in leukemia cells. The CAR construct is derived from antibodies identified in AML patients who achieved long-term remission following bone marrow transplantation (graft-versus-leukemia antibodies). These T cells are 'armored' to constitutively secrete interleukin-18 (IL-18), which induces interferon-gamma (IFNγ) to upregulate U5 snRNP200 surface expression on cancer cells. This creates a positive feedback loop, termed an 'antigen gain loop,' that enhances anti-tumor activity and immune activation while sparing healthy hematopoietic cells.
