Overview

IL-4-secreting Lactobacillus paracasei is an engineered live biotherapeutic product (LBP) designed for the treatment of acute lung injury (ALI). Utilizing synthetic biology, the probiotic strain Lactobacillus paracasei is engineered to locally deliver the anti-inflammatory cytokine interleukin-4 (IL-4). The therapeutic is further enhanced with a material-based surface modification that enables targeted accumulation in inflamed lung tissue and provides reactive oxygen species (ROS) scavenging capacity. In preclinical models of ALI induced by LPS and Pseudomonas aeruginosa, this dual-functional engineered probiotic has demonstrated the ability to suppress pro-inflammatory cytokines (TNF-α and IL-6), reduce intracellular ROS levels, and improve pulmonary histopathology. Additionally, metabolomic profiling indicates that the treatment remodels the pulmonary microenvironment by increasing host-beneficial metabolites such as indole-3-butyric acid.
