An IL-6 blocking antibody is a therapeutic monoclonal antibody designed to neutralize interleukin-6 (IL-6), a key pro-inflammatory cytokine involved in various autoimmune diseases and the tumor microenvironment. By binding directly to the IL-6 ligand, these antibodies prevent the formation of the IL-6/IL-6R/gp130 signaling complex, thereby inhibiting the JAK/STAT3 pathway. In oncology, IL-6 is recognized as a mediator of resistance to tyrosine kinase inhibitors (TKIs) and immune checkpoint inhibitors, particularly in EGFR-mutant non-small cell lung cancer (NSCLC). Preclinical data suggests that blocking IL-6 can restore T cell and NK cell anti-tumor activity and enhance the efficacy of PD-1/PD-L1 blockade. Clinically, this class includes agents like siltuximab, which is approved for multicentric Castleman disease, and others being investigated for their ability to overcome immunotherapy resistance and manage cytokine-related toxicities.
