**il-6 inhibitor + pd-l1 inhibitor** is a combination therapy consisting of an Interleukin-6 (IL-6) pathway inhibitor and a programmed death-ligand 1 (PD-L1) checkpoint inhibitor. The IL-6 inhibitor (commonly a monoclonal antibody targeting IL-6 or its receptor) works by blocking IL-6–mediated pro-inflammatory and immunosuppressive signaling in the tumor microenvironment. The PD-L1 inhibitor (often a monoclonal antibody targeting PD-L1) blocks the interaction between PD-1 on T cells and PD-L1 on tumor or immune cells, preventing immune evasion by tumor cells. This combination has been shown in preclinical models to enhance antitumor immunity by increasing tumor-infiltrating effector T cells (especially CD8^+^ T cells), shifting T cell phenotypes toward Th1, overcoming immune resistance, and resulting in superior tumor control compared to monotherapies. Primary indications under investigation include various solid tumors such as pancreatic ductal adenocarcinoma, hepatocellular carcinoma, renal cell carcinoma, and breast cancer[1][2][3][4][5].
