Overview

IL21120038 is a first-in-class, highly selective small molecule agonist of the C-X-C chemokine receptor 7 (CXCR7). Developed by iLeadBMS, a spin-off from Ildong Pharmaceutical Group, it is being investigated primarily for its antifibrotic and anti-inflammatory properties. CXCR7 is highly expressed in cardiomyocytes and plays a crucial role in preventing cell death during ischemic or damaged heart conditions. By activating CXCR7, IL21120038 reduces levels of the pro-inflammatory chemokine CXCL12 and indirectly inhibits the CXCR4-CXCL12 signaling pathway, which helps prevent fibrosis and inflammation. The drug also blocks migration of CXCR4+ T cells to inflamed tissues in autoimmune diseases, potentially reducing excessive immune responses. Preclinical studies have shown that IL21120038 significantly reduces infarct size and cardiac injury markers in rat models of myocardial infarction while improving cardiac function compared to standard therapies like sacubitril/valsartan (Entresto). It is under development for heart failure as well as various fibrotic and autoimmune diseases where current treatments are inadequate[1][5][7].
