ILB-202 is an engineered exosome-based therapeutic developed by ILIAS Biologics. It contains the anti-inflammatory protein super-repressor IκBα (srIκB), a stabilized and degradation-resistant form of the NF-κB inhibitor IκBα. The drug is designed to selectively inhibit excessive activation of nuclear factor κB (NF-κB), a key transcription factor involved in inflammatory and autoimmune diseases, by preventing its translocation into the nucleus in inflamed tissues. Unlike conventional NF-κB inhibitors that broadly suppress immune function, ILB-202 targets only cells with pathologically high NF-κB activity, reducing off-target effects and minimizing immunosuppression. Preclinical studies have shown efficacy in models of sepsis, kidney ischemia-reperfusion injury, chronic pain, rheumatoid arthritis, inflammation-induced preterm birth, and neuroinflammation. In phase 1 clinical trials with healthy volunteers, intravenous administration was safe and well tolerated without serious adverse events or immunogenicity[1][2][6].
