Iloprost + nitric oxide is a combination of two vasodilators used primarily in the management of pulmonary arterial hypertension (PAH) and acute pulmonary hypertension. Iloprost is a synthetic analogue of prostacyclin (PGI2), which acts as a potent vasodilator by mimicking endogenous prostacyclin to relax vascular smooth muscle and inhibit platelet aggregation. Nitric oxide (NO), administered as an inhaled gas, induces vasodilation by stimulating guanylate cyclase in vascular smooth muscle cells to increase cyclic GMP levels. Both agents lower pulmonary vascular resistance through distinct intracellular pathways—iloprost via cAMP and NO via cGMP—and are sometimes used together for additive or alternative effects in patients with PAH or during cardiac surgery. However, studies indicate that their combination does not provide significantly greater hemodynamic benefit than either agent alone[1][2][4][5].
