Overview

Ilunocadase (also known as recAP) is a recombinant human alkaline phosphatase developed by AM-Pharma for the treatment of sepsis-associated acute kidney injury (SA-AKI). The drug is a dephosphorylating enzyme that functions by neutralizing extracellular pro-inflammatory mediators such as lipopolysaccharide (LPS) and adenosine triphosphate (ATP), which are released during infection and cellular stress. By detoxifying these molecules, ilunocadase aims to reduce systemic inflammation and protect the renal microenvironment from damage. While early studies utilized bovine intestinal alkaline phosphatase (bIAP), AM-Pharma transitioned to the recombinant human version for clinical development. Despite reaching Phase 3 trials (REVIVAL study), the program faced challenges when the primary endpoint was not met in certain populations, though development in related indications continues.
