Imarikiren (TAK-272, also known as SCO-272) is a novel, potent, and orally active small molecule direct renin inhibitor developed for the treatment of hypertension, diabetic kidney disease, and heart failure. It acts by inhibiting renin—the rate-limiting enzyme in the renin–angiotensin–aldosterone system (RAAS)—thereby blocking the entire RAAS cascade upstream of angiotensin-converting enzyme inhibitors (ACE inhibitors) and angiotensin receptor blockers (ARBs). This mechanism may provide more effective blood pressure control and organ protection than downstream RAAS blockade. Imarikiren has demonstrated efficacy in preclinical models for reducing blood pressure, albuminuria in type 2 diabetes patients, and improving cardiac outcomes. Clinical studies have shown it to be safe and well-tolerated with good oral bioavailability[1][2][3][4][5][6].
