IMC-S118AI is a first-in-class, tissue-specific bispecific recombinant fusion protein developed by Immunocore. It is designed as a disease-modifying therapy for type 1 diabetes. The molecule targets pancreatic beta-cells by recognizing a peptide from pre-proinsulin presented by HLA-A*02:01 on the cell surface and delivers an effector arm that acts as a programmed cell death 1 (PD-1) receptor agonist. This dual targeting enables selective down-modulation of pathogenic T cells at the site of autoimmune attack in the pancreas, aiming to protect beta-cells from immune-mediated destruction while minimizing systemic immunosuppression. IMC-S118AI is part of Immunocore’s ImmTAAI platform for autoimmune diseases and represents an innovative approach to tissue-specific immune modulation[3][4][5][9].
