Drug intelligence / Profile preview
**Imipenem + relebactam + aztreonam** is an investigational combination antibacterial regimen consisting of the carbapenem antibiotic imipenem, the diazabicyclooctane beta-lactamase inhibitor relebactam, and the monobactam antibiotic aztreonam. This combination is being evaluated for the treatment of serious, multidrug-resistant (MDR) Gram-negative bacterial infections, particularly those caused by pathogens co-producing metallo-beta-lactamases (MBLs, such as NDM, VIM, or IMP) and serine beta-lactamases (such as KPC or AmpC). Aztreonam is uniquely stable against hydrolysis by MBLs but is vulnerable to serine beta-lactamases. Relebactam protects aztreonam from degradation by inhibiting class A and class C serine beta-lactamases, while imipenem provides additional broad-spectrum bactericidal activity against susceptible co-pathogens. The combination is typically administered as a co-administration of the approved fixed-dose combination imipenem/cilastatin/relebactam (Recarbrio) alongside aztreonam.
