Imlifidase is a recombinant cysteine protease derived from *Streptococcus pyogenes* that specifically targets and cleaves human immunoglobulin G (IgG). By cleaving the heavy chains of IgG at the hinge region, it effectively neutralizes IgG-mediated effector functions, such as complement-dependent cytotoxicity (CDC) and antibody-dependent cellular cytotoxicity (ADCC), within hours of administration. Developed by Hansa Biopharma, imlifidase is primarily indicated for the desensitization of highly sensitized adult kidney transplant patients who have a positive crossmatch against an available deceased donor. This rapid depletion of donor-specific antibodies (DSAs) enables successful transplantation in patients who would otherwise remain on waiting lists due to high immunological risk. Beyond transplantation, imlifidase is being investigated for the treatment of acute IgG-mediated autoimmune conditions, including anti-glomerular basement membrane (anti-GBM) disease, Guillain-Barré syndrome, and antibody-mediated rejection.
