ImmTAAI (Immune Modulating Monoclonal-TCR Against Autoimmune Disease) is a preclinical bispecific recombinant fusion protein developed by Immunocore for the treatment of type 1 diabetes. The molecule is designed as a tissue-targeted immunotherapy consisting of two functional domains: a high-affinity T-cell receptor (TCR) domain that specifically recognizes the pre-proinsulin (PPI) peptide (residues 15-24) presented by HLA-A*02 on the surface of pancreatic beta cells, and an effector domain comprising a PD-1 agonist (such as PD-L1). By selectively binding to beta cells, the ImmTAAI molecule concentrates PD-1 agonism at the site of autoimmune attack, delivering inhibitory signals to nearby autoreactive CD8+ and CD4+ T-cells. This localized mechanism of action aims to suppress the destruction of insulin-producing cells and reduce inflammatory cytokine secretion while avoiding the systemic immunosuppression associated with non-targeted therapies.
