This therapeutic procedure, developed at Northwestern University, involves autologous non-myeloablative hematopoietic stem cell transplantation (HSCT) for patients with severe, treatment-refractory systemic lupus erythematosus (SLE). The protocol begins with the mobilization of peripheral blood stem cells using cyclophosphamide and granulocyte colony-stimulating factor (G-CSF), followed by CD34+ cell enrichment. Patients then undergo an immune-ablative conditioning regimen consisting of high-dose cyclophosphamide and equine anti-thymocyte globulin (ATG) to deplete the existing autoreactive immune system. The harvested autologous stem cells are subsequently reinfused to facilitate hematopoietic recovery and immune reconstitution. The goal of this immune reset is to induce long-term disease remission and immune tolerance in patients who have failed standard immunosuppressive therapies.
