Immune globulin + ganciclovir is a combination therapy investigated for the treatment of sight-threatening cytomegalovirus (CMV) retinitis, particularly in patients with AIDS. The combination utilizes ganciclovir, a synthetic nucleoside analogue of 2'-deoxyguanosine that inhibits CMV replication by acting as a competitive inhibitor of dGTP incorporation into viral DNA via CMV DNA polymerase. The addition of immune globulin (IVIG) provides passive immunization through a pool of polyvalent IgG antibodies, which can neutralize CMV virions and provide immunomodulatory effects. This specific combination was studied by the American National Red Cross in a pilot trial to determine if it could provide a faster initial response and longer duration of remission compared to ganciclovir monotherapy in immunocompromised individuals.
