This immunomodulatory regimen is a multi-drug combination protocol developed by Aljazeera Hospital to improve Intracytoplasmic Sperm Injection (ICSI) outcomes in infertile women with autoimmune thyroiditis. The protocol involves a baseline treatment of thyroxine, low-dose aspirin, selenium, calcium, and vitamin D, supplemented by a specific immunomodulatory course. This course includes prednisolone (a glucocorticoid) to suppress systemic inflammation, hydroxychloroquine (an antimalarial with immunomodulatory properties) to reduce autoantibody levels, and azathioprine as an alternative for patients with hydroxychloroquine hypersensitivity. In cases non-responsive to these treatments, intravenous immunoglobulins are utilized. The regimen aims to mitigate the negative impact of thyroid autoantibodies on embryo implantation and pregnancy maintenance by modulating the maternal immune response.
