Overview

IN-119873 is a next-generation, orally available, and CNS-penetrable allosteric epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI) developed by HK inno.N. It specifically targets the EGFR L858R activating mutation and is designed to overcome resistance mechanisms associated with third-generation TKIs like osimertinib, particularly the EGFR C797S mutation. Unlike traditional ATP-competitive inhibitors, IN-119873 binds to an allosteric site separate from the ATP-binding site, providing high selectivity for mutant EGFR while sparing wild-type EGFR. This selectivity profile is intended to reduce common TKI-associated toxicities such as gastrointestinal and skin issues. Preclinical studies have demonstrated its efficacy in various osimertinib-resistant models and its ability to penetrate the blood-brain barrier, showing promise for treating intracranial metastases.
