INBRX-106 is a recombinant, humanized, hexavalent IgG antibody that acts as an agonist of the OX40 receptor (also known as TNFRSF4 or CD134), a costimulatory receptor expressed on immune cells and enriched in the tumor microenvironment. The drug is designed to bind and cluster six OX40 receptors simultaneously (hexavalency), promoting hyperclustering and potent activation of the OX40 costimulatory pathway without requiring Fc crosslinking. This mechanism enhances T-cell proliferation and function, reverses regulatory T-cell–mediated immune suppression, and amplifies antitumor immune responses. INBRX-106 is being developed primarily for use in combination with checkpoint inhibitors such as pembrolizumab for various solid tumors including head and neck squamous cell carcinoma (HNSCC), non-small cell lung cancer (NSCLC), triple-negative breast cancer (TNBC), melanoma, renal cell carcinoma, urothelial carcinoma, MSI/TMB-high tumors, among others[1][2][3][4][5].
