INCA035784 is a bispecific monoclonal antibody T-cell engager developed by Incyte for the treatment of myeloproliferative neoplasms (MPNs), specifically targeting patients with calreticulin (CALR) exon 9 mutations. This investigational therapy is designed to redirect T cells to malignant cells expressing mutant CALR, thereby sparing normal cells. Unlike earlier agents that may have variable activity across CALR mutation subtypes, INCA035784 demonstrates equipotent activity against all types of CALR mutations and binds a distinct epitope compared to other anti-mutant CALR antibodies in development. The drug is currently being evaluated as monotherapy in phase 1 clinical trials for safety, tolerability, and preliminary efficacy endpoints such as spleen volume reduction and symptom improvement in MPN patients[1][2][8].
