INCAGN01949 is a fully human immunoglobulin G1κ (IgG1κ) monoclonal antibody that acts as an agonist of the OX40 receptor (also known as tumor necrosis factor receptor superfamily member 4). It was developed to promote tumor-specific immunity by activating effector T cells and potentially depleting regulatory T cells (Tregs) through Fcγ receptor-mediated mechanisms such as antibody-dependent cell cytotoxicity (ADCC) and phagocytosis. By binding to OX40 on activated T cells, it aims to enhance their function and anti-tumor activity. Preclinical studies showed that it can facilitate OX40 clustering and downstream activation of the NFκB pathway in T cells, leading to increased production of pro-inflammatory cytokines like TNFα and IFNγ while decreasing IL-10. Despite promising preclinical data, clinical trials in advanced or metastatic solid tumors demonstrated limited pharmacodynamic effects on peripheral blood or intratumoral T-cell activation/function. No significant safety concerns were observed in monotherapy studies[1][2][5][6][8][10].
