Overview

INCAGN1876 (ragifilimab) is a human IgG1 monoclonal antibody that acts as an **agonist** of **glucocorticoid-induced TNFR-related protein (GITR, also known as CD357 or TNFRSF18)**. It is specifically designed to engage a conformational epitope on the extracellular domain of GITR, promoting receptor clustering and robust downstream signaling on T cells. The mechanism of action includes: (1) co-stimulatory activation of T cells to enhance anti-tumor immune responses, (2) potential facilitation of T cell priming and activation both with and without antigen-presenting cells (APCs), and (3) selective depletion of activated intratumoral regulatory T cells. INCAGN1876 has demonstrated greater activity on an IgG1 Fc backbone, leveraging Fcγ receptor-mediated clustering and effector functions. It is under development as an immunotherapy for various advanced or metastatic solid tumors, often in combination with other immune checkpoint inhibitors such as anti-PD-1 (nivolumab) or anti-CTLA-4 (ipilimumab). The drug was originally discovered in a collaboration with Ludwig Cancer Research and is being co-developed by Agenus and Incyte[1][2][3][7][5].
