INCB086550 is an orally available, small molecule inhibitor of programmed cell death ligand 1 (PD-L1; CD274), developed as a first-in-class oral immune checkpoint inhibitor. By binding to PD-L1 on tumor cells, it blocks the interaction between PD-L1 and its receptor programmed cell death protein 1 (PD‑1; CD279) on T-cells. This blockade reverses T-cell inactivation caused by the PD‑L1/PD‑1 pathway, leading to increased T-cell expansion and enhanced cytotoxic anti-tumor immune responses. Preclinical studies have shown that INCB086550 induces PD-L1 dimerization and internalization, stimulates cytokine production in primary human immune cells, activates T-cells, and reduces tumor growth in animal models. Early clinical trials indicate that its safety profile is similar to antibody-based checkpoint inhibitors but with potential advantages such as oral administration and distinct pharmacology. It is being investigated for use in various solid tumors including non-small cell lung cancer, renal cell carcinoma, urothelial carcinoma (bladder cancer), hepatocellular carcinoma (liver cancer), and melanoma.
