INCB161734 is a novel, potent, selective, and orally bioavailable small-molecule inhibitor targeting the KRAS G12D mutation. Developed by Incyte, it is designed for the treatment of advanced and metastatic solid tumors harboring the KRAS G12D mutation, including pancreatic ductal adenocarcinoma (PDAC), colorectal cancer (CRC), and non-small cell lung cancer (NSCLC). Preclinical studies have demonstrated that INCB161734 achieves strong target engagement with near-maximal inhibition of KRAS signaling in tumor models, resulting in significant tumor growth inhibition or regression. The drug disrupts cell cycle progression and induces apoptosis specifically in KRAS G12D mutant cells while sparing wild-type KRAS cells. It is currently being evaluated as both monotherapy and in combination with other anticancer agents such as cetuximab, retifanlimab, GEMNabP (gemcitabine plus nab-paclitaxel), and mFOLFIRINOX[2][3][4][6].
