Inclacumab is a fully human IgG4 monoclonal antibody that selectively inhibits P-selectin, a cell adhesion molecule expressed on activated endothelial cells and platelets. By blocking P-selectin, inclacumab disrupts the interactions between sickled red blood cells, leukocytes, platelets, and the endothelium that contribute to vaso-occlusive crises (VOCs) in sickle cell disease (SCD). The drug was originally developed by Roche/Genentech for cardiovascular indications such as myocardial infarction and peripheral arterial disease but is now being advanced by Pfizer (following its acquisition of Global Blood Therapeutics) primarily for SCD. Inclacumab’s mechanism of action is similar to crizanlizumab but binds a different region of P-selectin and may allow less frequent dosing. It has orphan drug designation for SCD and has reached phase III clinical trials in this indication[1][3][5][8].
