Indium-111 anti-CEA monoclonal antibody is a radiolabeled chimeric or murine IgG1 monoclonal antibody directed against carcinoembryonic antigen (CEA), a tumor-associated glycoprotein highly expressed in many colorectal and other carcinomas. The drug consists of an anti-CEA monoclonal antibody (such as cT84.66 or ZCE-025) conjugated to the radioisotope indium‑111 via chelators like diethylenetriaminepentaacetic acid (DTPA) or DOTA for use in radioimmunoscintigraphy imaging of CEA-expressing tumors[1][4][5]. Upon intravenous administration, the labeled antibody binds specifically to CEA on tumor cells; the gamma emissions from Indium‑111 allow for external imaging and localization of primary and metastatic lesions using nuclear medicine techniques[1][4]. This approach enables detection of otherwise occult disease sites in patients with suspected recurrence or metastasis of CEA-producing cancers such as colorectal cancer[4][5]. The mechanism is based on specific binding to cell-surface CEA followed by visualization through radiotracer emission.
