Indium-111-DOTA-girentuximab-IRDye800CW is a dual-modality imaging agent consisting of the chimeric monoclonal antibody girentuximab conjugated with both the radioisotope indium-111 and the near-infrared fluorescent dye IRDye800CW. This agent specifically targets carbonic anhydrase IX (CAIX), which is highly expressed in more than 95% of clear cell renal cell carcinomas (ccRCC). The dual-labeling approach allows for both preoperative imaging with SPECT/CT (using the radionuclide component) and intraoperative fluorescence imaging (using the fluorescent dye component), overcoming the limited penetration depth of light in biological tissue. This enables surgeons to initially locate tumors using radiodetection and then precisely delineate tumor margins using fluorescence imaging during surgical resection.
