Individual Patient TCR-Transduced PBL is a form of adoptive cell therapy in which a patient's own peripheral blood lymphocytes (PBLs) are genetically engineered to express a tumor antigen-specific T-cell receptor (TCR). The process involves isolating PBLs from the patient, identifying tumor-specific or neoantigen-reactive TCRs—often derived from the patient's own tumor-infiltrating lymphocytes or by screening for reactivity to mutated antigens such as mutant p53—and then using retroviral vectors to transduce these TCR genes into the PBLs. The modified cells are expanded and reinfused into the patient following non-myeloablative lymphodepleting chemotherapy. This therapy aims to direct an immune response specifically against cancer cells expressing target antigens. It has been investigated primarily for metastatic epithelial cancers such as mismatch repair-proficient colorectal cancer and other solid tumors that have progressed after standard therapies. Clinical trials have shown early signs of efficacy in patients with advanced disease[3][4][5][8].
