**Indoximod + ipilimumab** is a combination therapy studied for advanced melanoma, consisting of indoximod, a small molecule inhibitor targeting the indoleamine 2,3-dioxygenase (IDO) pathway, and ipilimumab, a monoclonal antibody checkpoint inhibitor targeting cytotoxic T-lymphocyte–associated protein 4 (CTLA-4). Indoximod reverses immunosuppression mediated by IDO activity by countering depletion of tryptophan and accumulation of kynurenine in the tumor microenvironment, thereby enhancing T-cell–mediated antitumor immune responses. Ipilimumab blocks CTLA-4, thus reducing inhibitory signaling on T-cells and promoting immune activation against tumors. This combination was tested in a multi-arm phase 1/2 trial for metastatic and unresectable melanoma, establishing the indoximod phase 2 dose at 1200 mg twice daily oral administration, combined with standard-dose intravenous ipilimumab[3][5]. Treatment was administered concurrently and continued until disease progression or unacceptable toxicity[5].
