This is a combination therapy consisting of four biologic agents—**infliximab**, **adalimumab**, **ustekinumab**, and **vedolizumab**—each with distinct mechanisms of action targeting different inflammatory pathways. All four are monoclonal antibodies used primarily in the management of moderate to severe inflammatory bowel diseases (IBD), such as Crohn’s disease and ulcerative colitis, particularly in patients who have not responded adequately to monotherapy or conventional treatments. - Infliximab and adalimumab are anti-TNF agents that suppress inflammation by binding to tumor necrosis factor-alpha (TNF-α). - Ustekinumab inhibits the pro-inflammatory cytokines interleukin-12 (IL-12) and interleukin-23 (IL-23). - Vedolizumab blocks the α4β7 integrin, preventing T-cell migration into gut tissue, thereby reducing intestinal inflammation[2][3][6]. Combination regimens involving two or more of these agents have been explored for refractory IBD cases, but quadruple combinations like this are extremely rare in clinical practice due to safety concerns such as increased risk of infections[1][8]. The rationale for combining these drugs is to achieve synergistic inhibition across multiple immune pathways when single-agent therapy fails.
