Overview

This is a combination therapy of two monoclonal antibodies, infliximab and basiliximab, used primarily as salvage treatment for severe, steroid-refractory graft-versus-host disease (GvHD), particularly with gastrointestinal involvement. Infliximab is a chimeric monoclonal antibody that targets and neutralizes tumor necrosis factor alpha (TNF-alpha), a key pro-inflammatory cytokine implicated in various autoimmune and inflammatory conditions. Basiliximab is a chimeric monoclonal antibody that binds to the interleukin 2 receptor alpha subunit (CD25) on activated T lymphocytes, blocking IL-2-mediated activation of lymphocytes and thus suppressing immune responses. The combination aims to provide broader immunosuppression by targeting both TNF-alpha-driven inflammation and IL-2-dependent T-cell activation pathways. This regimen has been studied in patients with severe GI acute GvHD after allogeneic stem cell transplantation who are refractory to steroids[1][7][2]. It has also been explored in other settings such as steroid-refractory Crohn's disease[3].
