The influenza vaccine-Herlev Hospital-pancreatic cancer refers to the experimental use of a standard seasonal inactivated influenza vaccine as an immunomodulatory agent in the Phase II INFLUENCE trial (NCT05116917). Conducted at Copenhagen University Hospital—Herlev and Gentofte, the study evaluated the vaccine's potential to enhance systemic T-cell responses in patients with refractory metastatic pancreatic cancer. The vaccine was administered as a single intramuscular dose in combination with the immune checkpoint inhibitors nivolumab (anti-PD-1) and ipilimumab (anti-CTLA-4), alongside stereotactic body radiotherapy (SBRT). The therapeutic hypothesis was that the robust immune activation typically elicited by the influenza vaccine would convert the "cold" pancreatic tumor microenvironment into an "immunologically hot" one, thereby sensitizing the tumor to checkpoint inhibition. However, the trial results indicated that the combination did not demonstrate objective clinical responses or significant antitumor efficacy, although it maintained a manageable safety profile.
