Ingenol disoxate is a synthetic small molecule derivative of ingenol developed as a topical gel for the treatment of actinic keratosis and other skin disorders. It was designed to improve upon the chemical stability and potency of earlier compounds such as ingenol mebutate. Ingenol disoxate acts primarily by stimulating protein kinase C (PKC), leading to direct cytotoxicity in abnormal keratinocytes and induction of proinflammatory mediators that contribute to lesion clearance. Preclinical studies demonstrated increased cytotoxic potency and antitumor effects compared to ingenol mebutate. The drug was investigated for use in actinic keratosis and acne vulgaris but development has been discontinued[1][3][4][6].
