Overview

Ingenol mebutate + MAL PDT is an investigational combination treatment regimen being studied for the treatment of actinic keratosis. It pairs topical ingenol mebutate (Picato), a protein kinase C (PKC) activator derived from the plant *Euphorbia peplus*, with methyl aminolevulinate photodynamic therapy (MAL PDT). Ingenol mebutate induces rapid lesion necrosis and an immune-mediated response, while methyl aminolevulinate acts as a prodrug photosensitizer that, upon light activation, generates reactive oxygen species to destroy dysplastic keratinocytes. This sequential or combined approach is being evaluated to improve clearance rates and patient outcomes in actinic keratosis on the scalp and face, particularly in a Phase 2 study led by Pacific Dermaesthetics in collaboration with LEO Pharma.
