Overview

Inhaled human insulin + pioglitazone is a combination therapy intended for the management of type 2 diabetes mellitus, where glycemic control is insufficient with monotherapy. Inhaled human insulin is a fast-acting insulin analogue that provides rapid reduction in postprandial blood glucose by facilitating cellular glucose uptake. Pioglitazone is a thiazolidinedione (TZD) that acts as an insulin sensitizer, primarily by activating the peroxisome proliferator-activated receptor gamma (PPARγ), thereby enhancing peripheral glucose uptake and decreasing insulin resistance. The combination may improve both fasting and postprandial glycemia, and has been associated with reductions in HbA1c. Combination use increases the risk of hypoglycemia and edema, and may lead to a reduction in required insulin doses. Inhaled insulin is not recommended for patients with chronic lung disease, and pioglitazone may increase the risk of bone fractures, edema, and heart failure, particularly in those predisposed. There are no known commercial fixed-dose combination products of inhaled human insulin and pioglitazone; any such combination would be a co-administration of individually approved agents[1][2][3].
